I hereby grant to Collegiate Sports Management Group (CSMG) and its divisions of EsportsU, the Collegiate Esports Commissioners Cup (CECC), and any of its partner’s permission to use any audio, photo, and/or video taken of myself and/or my child (if completing this form as a parent guardian for a depended under 18 years of age). I hereby grant CSMG permission for non-exclusive rights to use the videos, written work(s), image(s), and/or art object(s) described below on CSMG’s media platforms, linear outlets, and social media platforms for any CSMG distribution.
I hereby certify and covenant that I am of legal age (a parent or legal guardian must sign for a minor), or if applicable, that I am authorized to sign on behalf of the entity listed below.
